Multiple calls to renderSpan.end() violate OpenTelemetry specification

What fails: The renderSpan in renderToResponseWithComponents() is ended multiple times - via explicit endSpanWithError() calls in catch blocks AND via .finally() handlers attached to stream promises, violating OpenTelemetry's contract that "only the first call to end should modify the span."

How to reproduce:

  1. Trigger an error during app rendering that reaches the outer catch block (line 2957)
  2. This causes endSpanWithError() to be called (lines 2969, 2982, 3048)
  3. Later when htmlStream.allReady or fizzStream.allReady promises settle, their finally handlers execute
  4. Each finally handler calls renderSpan.end() again (lines 2928, 3091)
  5. This results in 2-4 calls to renderSpan.end() on the same span

Expected behavior: According to OpenTelemetry Python documentation, "Only the first call to end should modify the span, and implementations are free to ignore or raise on further calls." The code should ensure the span is ended exactly once.

Implementation: Added a spanEnded flag to track span state and created helper functions endSpanWithError() and endSpan() that check this flag before calling renderSpan.end(). This ensures the span is only ended once, regardless of whether the error path or the finally handler executes first.

What fails: The endSpanWithError() function in packages/next/src/server/app-render/app-render.tsx (lines 2581-2592) only sets the span's ERROR status when the caught error is an Error instance. When non-Error values are thrown (e.g., throw "string"), the span is ended without calling setStatus(), so it defaults to UNSET status instead of ERROR.

How to reproduce: In any catch block that calls endSpanWithError() in the app render flow, throw a non-Error value:

This is consistent with the closeSpanWithError() function in packages/next/src/server/lib/trace/tracer.ts (line 55-66), which always calls setStatus({ code: SpanStatusCode.ERROR }).

Fix: Move the setStatus() call outside the if (err instanceof Error) check and use optional chaining to safely handle the message property for non-Error values.
